4. Dateofpurchase

5. ProcedureindexandpagenumberinwhichthefailingFRUwasdetected

6. FailingFRUnameandpartnumber

7. Machinetype,modelnumber,andserialnumber

8. Customer’snameandaddress

Note for warranty:

During thewarrantyperiod,thecustomermaybe

responsiblefor repaircostsif thecomputerdamage wascausedbymisuse,

accident,modification,unsuitablephysical oroperatingenvironment, or

impropermaintenancebythecustomer.
